What are vendor questionnaires?

Collect and document vendor information in a structured and consistent way.

Vendor questionnaires help you gather information from your vendors as part of audits or evaluations.

In Cerivo, you can send questionnaires directly to vendors, track responses, and store everything in one place.


Where to find questionnaires

You can find questionnaires on each vendor’s page under the Questionnaires tab.

Here, you can:

  • view all questionnaires sent to the vendor
  • track their status
  • download responses

How to send a questionnaire

To send a questionnaire:

  1. Open the vendor
  2. Go to the Questionnaires tab
  3. Select Add questionnaire

You’ll then complete the setup:

Basic information

  • Select the language
  • Choose the questionnaire
  • Enter the recipient’s name and email
  • Select the sending company (if applicable)

Email message

You can review and adjust the email sent to the vendor.

You can also save your message as a default template for future use.

Once ready, select Send.


What happens next

After sending:

  • The vendor receives an email with a link to the questionnaire
  • Cerivo automatically sends reminders every two weeks
  • After three reminders, the questionnaire is marked as Unanswered

You’ll receive a notification when:

  • the vendor completes the questionnaire
  • the questionnaire is marked as unanswered

This means you don’t need to manually follow up.


Customizing questionnaires

Cerivo includes a set of standard questionnaires that cover common audit needs.

If you require custom questionnaires, this can be configured depending on your plan.
